The PFG Account Papers are being provided on-line for your convenience. If you would prefer to have the account documents sent to you per U.S. mail, please contact a customer service representative at (800)333-5673 or (312) 775-3000.

All documents required to open a traditional or electronic trading account are provided here. Please feel free to review them at your computer, or download to your printer for completion.Select the document(s) that apply to you. (If downloading documents, please allow adequate time to complete the process.The amount of time required to download the documents depends upon your internet connection speed.)

Before completing the forms, simply refer to the PFG Account Matrix (page 3 of the Customer Agreement) to determine which forms you will be required to complete. Make sure to keep a copy for your records. Upon completion, forward THE COMPLETE SET of forms and the TRANSMITTAL MEMORANDUM to:

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.

Upon receipt, a customer service representative will be in contact with you. If you have any questions regarding the forms, please contact a customer service representative at (800) 333-5673 or (312) 775-3000. If you are already working with a PFG Broker, please contact the broker directly for instructions.

Document Format

PFG Customer Account Agreement documents are stored in PDF format. PDF is a way to view and print documents specifically designed for electronic distribution. PDF documents will view and print correctly regardless of your type of computer or operating system.

Account Transfer This form is used when a customer needs to transfer money or positions from another firm to PFG.

Auto Liquidation Acknowledgement  

Canadian Non Solicit Required for Canadian Citizens.

Complete Set Complete set of Account documents:
English English Account Opening Documents or Deutsch German Account Opening Documents.

Corporate Own Funds  

Email Statement Required for customers who wish to receive their daily and monthly statements via email.

Letter Of Direction The Letter of Direction is a requirement for those customers who have a broker that will trade a third-party trading system.

Own Funds Required for customers that indicate their occupation may be investment related.

POA Forms Required for customers that indicate a third party will be trading the account on behalf of the customer. Customer will not be notified of the trades as they are placed.

Second Account Request Letter Required for customers that want more than one account.

W-8BEN Certificate of  Foreign Status The W-8BEN form is required for all foreign customers.
